1.  Registration Deadline:  11:55 p.m. on October 6, 2023   

    NO EXCEPTIONS. NO REFUNDS AFTER DEADLINE.

 

2.  ALL completed Fall Forum Registration lists MUST be received by the deadline.

3.  I will send you an invoice to submit for payment, once I receive your list of attendees.  Mail checks to the address below.       

    Advance receipt of checks is appreciated.

4.  Pin/T-shirt Entries may be submitted to Shirlene Bender via email (no text please) by the Chapter Advisor or the submitting student.   Encourage this!  The name of the designer will have their name & school printed on the back of the t-shirt.  There will be prizes for:  winning t-shirt and pin, and also for 2nd & third place designs.  They MUST read the rules for designs. Any designs not following listed rules will not be considered.

5.  Chapters are asked to send in a Chapter Photo via email to Shirlene Bender.  Get your chapter together and get someone to   

    take a picture!  You get HOSA Cup points for this!  Try to get everyone in the same uniform or shirt.  We will be showing these   

    chapter pictures at FFLC or SLC.

6.  It is strongly recommended that all in attendance stay for the whole time on that day in order not to miss any important information.

​

7.  Have a student who can really sing?  Have them submit a sample of them singing the national anthem to Shirlene Bender.

     One person will be chosen to sing, and one chosen to be the alternate.
